New Mexico Medicaid Billing

New Mexico expanded Medicaid under the ACA effective January 1, 2014, with then-Governor Susana Martinez accepting expansion as ‘what is best for New Mexicans.’ The state’s Medicaid program (Centennial Care, now Turquoise Care) covers approximately 800,000–850,000 members in managed care plus an additional 160,000+ in fee-for-service. New Mexico has one of the highest Medicaid participation rates in the nation, reflecting the state’s relatively high poverty rate and large uninsured population. Turquoise Care operates under a 5-year 1115 demonstration waiver (through December 2028).

Turquoise Care MCOs (Effective July 1, 2024)

Four managed care organizations serve Turquoise Care members under contracts through December 2026 (with optional renewals to December 2028):

  • Blue Cross Blue Shield of New Mexico (BCBSNM): Incumbent from Centennial Care; locally significant insurer; serves both Medicaid managed care and commercial insurance in New Mexico
  • Presbyterian Health Plan (PHP): Incumbent; uniquely positioned as both a Medicaid MCO and the insurer arm of Presbyterian Healthcare Services (New Mexico’s largest health system); one in three New Mexicans has some relationship with Presbyterian
  • Molina Healthcare of New Mexico: New entrant to Turquoise Care; national Medicaid managed care operator with strong presence in New Mexico ACA marketplace; initially threatened to leave New Mexico if not selected for Centennial Care 2.0 (did not come to pass)
  • UnitedHealthcare Insurance Company: New entrant to Turquoise Care; largest national Medicaid managed care operator; replaced exiting Western Sky Community Care (Centene)

Western Sky Exit / Turquoise Care Transition

Western Sky Community Care (Centene’s New Mexico subsidiary) was not selected for Turquoise Care and exited June 30, 2024. Key transition:

  • Special open enrollment ran April 1–May 31, 2024 for Western Sky members to choose a new plan
  • Western Sky members who did not choose were auto-assigned to BCBSNM, PHP, Molina, or UnitedHealthcare
  • Providers contracted only with Western Sky needed to credential with new Turquoise Care MCOs before July 1, 2024
  • Pharmacy, durable medical equipment, and EPSDT prior authorizations were honored for 90 days after June 30, 2024

Native American Population — Special Provisions

New Mexico has one of the largest Native American populations of any state (approximately 11% of the population). Key billing considerations:

  • Native American Medicaid members may opt out of Turquoise Care managed care at any time and receive FFS coverage
  • Turquoise Care MCOs are required to have staff trained in Native American culture and traditions and provide culturally competent care
  • Indian Health Service (IHS) and Tribal 638 facilities have distinct billing rules—IHS claims route separately from MCO claims
  • Native American members opting out of managed care are billed FFS to the New Mexico Health Care Authority (HCA)

  • No co-pays: New Mexico Medicaid does not require any co-pays from members—billing for cost-sharing is not permitted
  • Foster care managed care: Children in state custody are mandatorily enrolled in a specialized foster care plan within Turquoise Care
  • Verify plan enrollment at every visit—membership may have changed during the April–May 2024 Turquoise Care open enrollment
  • Provider enrollment: New Mexico HCA (formerly HSD) through the YES.NM.GOV portal; credential separately with each MCO
  • Turquoise Care helpline: 1-800-283-4465
  • Timely filing: Varies by MCO—typically 180–365 days; FFS timely filing is 12 months for non-managed care populations

Presbyterian Healthcare Services

Albuquerque-based Presbyterian Healthcare Services is New Mexico’s largest health system with NPR of $1.7 billion and nine hospitals statewide. Founded as a small tuberculosis sanatorium in the early 20th century, Presbyterian has grown into a fully integrated delivery system serving one in three New Mexicans with either coverage or healthcare. Presbyterian Hospital (453 beds, Albuquerque)—the system’s flagship—has NPR of $1.38 billion and is ranked #1 in New Mexico by U.S. News & World Report. Uniquely, Presbyterian Health Plan serves as both New Mexico’s largest health insurer AND one of the four Turquoise Care Medicaid MCOs, creating deep vertical integration across the payer-provider continuum. Presbyterian is attempting to construct an 11-story patient care tower to bring private room capacity to 600 beds.

UNM Health System

The University of New Mexico Health System is New Mexico’s academic health center with NPR of $1.4 billion and five affiliated hospitals. UNM Hospital (Albuquerque, 530+ beds) is the state’s second-largest hospital by NPR at $1.3 billion and holds critical designations: New Mexico’s only Level I Trauma Center, the state’s only academic medical center, the only dedicated children’s hospital (Children’s Hospital of New Mexico), and the primary teaching hospital for UNM School of Medicine. UNM Health provides 900,000+ outpatient visits, 22,000 surgical cases, and 100,000 ED visits annually, serving as New Mexico’s safety-net referral center for the state’s most complex and uninsured patients.

Lovelace Health System

Albuquerque-based Lovelace Health System has NPR of $720 million and six hospitals, including unique specialty facilities: the only hospital in New Mexico dedicated exclusively to women’s health (Lovelace Women’s Hospital) and the only hospital in New Mexico providing exclusively cardiovascular care (Heart Hospital of New Mexico at Lovelace Medical Center). Lovelace is part of a for-profit system and serves the Albuquerque metro’s commercial and Medicaid managed care populations alongside Presbyterian and UNM. Lovelace Medical Group/New Mexico Heart Institute operates a large employed physician group across Albuquerque.
